It's common, and totally understandable, to have a lot of questions when you're considering starting hormone replacement therapy (HRT).
Luteinizing hormone (LH) and follicle-stimulating hormone (FSH), released from the pituitary gland, are the hormones responsible for stimulating ovarian egg development, and ovulation. Some people experience regular bleeding or spotting even after years of using testosterone. However, just under 1 in 4 people (25%) report that it takes longer than six months for their period to stop.
In order to observe a 50% increase in testosterone levels between day 4 and 14, with a standard deviation of 40%, and a power of 0.99, a sample size of 25 pairs was needed (using unpaired power calculation and data from a previous study ). The experimenter performing the analysis was blinded to the participants' menstrual cycle data, and since the samplings had started in random time-points in the cycle, the tests' analysis plate distribution was random. The information letter stated that the trial concerned how testosterone concentrations were affected by different social conditions, but the main hypothesis was not disclosed to the participants during the entire study. It is also questionable whether our original hypothesis for the current study was correctly stated, or if e.g. other days than day 4 versus ovulation day should be the main comparison.
JOS handled all contacts with the participants recruited in Linköping, participated in the testosterone analysis and drafted the manuscript. Before analysis, each testosterone concentration was normalized by dividing by the highest concentration for every specific individual. For detection of ovulation, hCG-tests for home use were provided (Wondfo hCG Urine Test, Cat. No. W1 - MII, Guangzhou Wondfo Biotech Co, Guangzhou, China). After the sampling period, all samples were collected by the research team, and stored in -20°C until analysis. The saliva samples were immediately put in a plastic bag and stored in the home freezer until the end of the sampling period.
In ascertaining serum T levels, it is necessary to measure both the free testosterone (FT) and the albumin bound fractions. In females only 1% of T remains free and the rest is bound to proteins like sex hormone binding globulin (SHBG), albumin and cortisol binding globulin, reducing its bio-availability . Ovarian androgen production and conversion of T into estradiol are essential to the physiological process of ovulation and insufficient androgen production in the follicular phase can lead to anovulation . Ovarian synthesis of testosterone occurs in theca cells and is mainly regulated by luteinizing hormone (LH) and also by insulin. In females, approximately 0.02 micromoles of T is secreted per day from the theca cells of the ovaries and the adrenal cortex .
Sometimes a doctor will recommend an internal pelvic ultrasound (also known as transvaginal ultrasound), where the images are collected by inserting an ultrasound tool into the vagina/front hole. For this reason, it’s best to chat to a doctor about your individual health. However, it’s important to remember that testosterone isn’t a reliable contraceptive, and pregnancy may still be a possibility. If the egg is not fertilized with a sperm, the egg will die and it is shed from the uterus with the lining – this causes menstruation, or a period. Future study protocols should include at least three sampling points per MC as a peak cannot be demonstrated when plotting only two measurements.
This includes studies that have demonstrated that males perceive near-ovulation odors as more pleasant than odors from women far from ovulation 20,21. A number of exogenous factors, such as lacrimal secretions, can influence the male testosterone concentrations , possibly reflecting pheromonal effects. To further investigate your symptoms, a clinician may want to get a blood test to check your testosterone levels. If menstrual symptoms cause gender dysphoria for you, we recommend you seek out mental health support from friends, family, and/or a therapist.
While immediate therapy isn't always necessary, a balanced diet and natural supplements can effectively manage these levels. Regular testosterone tests are crucial for understanding these imbalances. That's because testosterone's psychological effects, such as improved mood and reduced stress, can enhance athletic performance and motivation during training sessions. I recommend light exercise, including strength training, which has proven beneficial for both pain relief and overall hormonal balance during this phase.
